Registering with us as a Patient

We are open for new registrations and are particularly delighted to welcome families registering together. We accept patients from Haverhill and many surrounding villages. The map below shows the 19th-century boundaries in this area. If you are on our map we can usually accommodate you. We require 2 forms of ID when registering (preferably photographic and proof of your current address). Unfortunately, we can no longer register patients from Wickambrook, Denston, Clare, Ashen or Ridgewell.

Pre-Registration with the practice.

 

If you wish to pre-register with us, please click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time, you will be asked to sign the form to confirm that the details are correct. You will also need to bring 2 forms of ID with you(photographic and proof of your current address).

When you register, you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you can fill out and send to us. When you come to the surgery, you will be asked to sign this form to confirm that the details are correct.
