Social Prescribing Form

 

Social prescribers support individuals focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They can connect people to community groups and statutory services for practical and emotional support, and offer one to one support either face to face or over the phone. Social prescribers, although part of your GP Surgery are not clinically trained, but can support individuals to take greater control of their health and wellbeing. Social prescribing works for a wide range of people, including people:

* with one or more long-term conditions

* who need support with their mental health

* who are lonely and isolated

* who have complex social needs which affect their wellbeing

 

This service is available to all adults over 18 and who are registered with the Surgery. Anyone who wishes to access our Social Prescribing Service can complete this self-referral form and it will forwarded to Abi, our Social Prescriber, who will contact you to offer assistance or arrange an appointment. Appointments can take place face to face or over the telephone.
